Select Attribute Form

When invoked from the Add button, the dialogue is displayed as follows:

All possible attributes for this entity are displayed and are grouped by category.

The grid is multi-select using the {Ctrl} and {Shift} keys with the mouse.

The Apply button assigns the attributes to the underlying record.

Cancel closes the form without assigning any attributes.
